Susan Hatfield graduated from St. Vincent’s Medical Center School of Nursing in New York and attended the College of Staten Island, CUNY for Nursing and is a Registered Nurse. She went on to obtain her Nursing Home Administrators License after attending George Washington University for Administration for Nursing Home Administrators. She is a Certified Developmental Disabilities Nurse and a Certified Case Manager as well.
Susan has experience in the healthcare industry working and obtaining compliance with external regulatory mandated requirements from CMS, Medicare, and Medicaid. I have a proven history with meeting these requirements while promoting customer satisfaction and continually pursuing excellence. All the while providing cost effective efficient care obtaining optimal outcomes. Her successful management experience in various healthcare fields has included the startup of several innovative programs for the IDD, which are still viable today. This demonstrates her commitment to excellence through expert leadership, managing resources and providing excellent customer service while achieving organizational goals while providing person-centered care.
Susan has over 20 years’ experience working with individuals with Intellectual and Developmental Disabilities most who had a Mental illness. She opened and managed, as the Regional Director of a Supportive Living facility serving IDD in Tennessee. Susan has worked in 28 States serving as a consultant and operations manager including Arkansas. Susan worked closely with BCBS of Arkansas and provided RFP response for the PASSE, Arkansas Advanced Care. She developed processes, policies & procedures for the Provider-Led Arkansas Shared Savings Entity and was instrumental in BCBS who was awarded a contract by State of Arkansas which scored 100% on readiness review with State of Arkansas.
She has served as SME for Intellectual and Developmental Disabilities population in the development of several programs.
Susan’s skills include Clinical Operations and Development, Case Management, Utilization Management, DSNP, Business Development, Program Implementation, Strategic Planning, Clinical and Financial Data Analysis, Team Building and Coordination, Customer Satisfaction, Process Improvement, Outcome Oriented, Transitions of Care, LTSS for individuals with Intellectual and Developmental Disabilities.
Susan is the Founder and President of the West Tennessee Developmental Nurse’s Network (WTDDNN), a Member of Developmental Disabilities Nurse’s Association (DDNA), a member of the National Association of Dual Diagnosis (NADD) and a Board Member of Tennessee National Association of Dual Diagnosis (TNADD) which shows her commitment to the IDD population.
Susan is the Vice President of Operations and Business Development for Achievement Health Group, LLC and will be responsible for oversight of the day-to-day operations of the CSSP program. She recently achieved the CQL Systems Accreditation for AHG.
